Please complete and review the following documents for your enrollment process.
In order to start or update your child's file, we need a completed enrollment packet and an immunization record. The enrollment packet must be submitted by April 15th. Your immunization record can be turned in later to allow for summer appointments. Immunization records must also show that your child has had a physical. This proof of physical is found in the box at the top right of the state physical form and must be signed and dated. We have included a blank document that you can either share with your doctor or review as a reference for what is needed.
