Please complete and review the following documents for your enrollment process.
In order to start or update your child's file, we need a completed enrollment packet and an immunization record. The enrollment packet must be submitted by April 15th. Immunization records must show that your child has had a physical. This proof of physical is found in the box at the top right of the state physical form and must be signed and dated. We have included a blank document that you can either share with your doctor or review as a reference for what is needed.
In the "For Your Review" section, there are documents that are referred to in your enrollment paperwork.
